Description
Working in collaboration with the physician group, the Advanced Practice Provider will assess and treat patients and engage in independent decision making about their health care needs. This is a full-time position working M-F shifts depending on location, 7:30am-5:00pm , 8:30am-5:00pm, and 8:30am-6:00pm and rotating Saturdays 8:30am-12:00pm. This role will support our St. Paul, Maplewood, and Eagan clinics. Pediatric and Young Adult Medicine is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.
Essential Functions:
- Perform complete, detailed and accurate histories
- Review patient records to develop comprehensive medical status reports
- Order laboratory, radiology and diagnostic studies appropriate for complaint
- Complete physical examinations and record pertinent data
- Interpret and evaluate patient data to determine patient management and treatment
- Make medical diagnoses and institute therapy and/or referrals
- Provide instruction and guidance regarding medical care matters to patients
- Complete review of outside records and complete paperwork as needed for patients. Return phone calls for lab and radiology tests. These tasks may be performed outside of clinic hours.
- Occasionally participate in community functions relating to PYAM outreach events throughout the year
- Any and all other duties as assigned
Education and Experience Requirements:
- Successful completion of advanced practitioner training and certification as a practitioner
(general or specialized) from an approved nurse practitioner or physician assistant program. - Current CPR certification.
- Graduate of an accredited Physician Assistant or Nurse Practitioner program
- NCCPA certification
- Minnesota Board of Medical Practice registration/license
- DEA registration
- Experience pediatric medicine preferred
- Comfortable with email, phone and instant messaging for patient and staff interaction

- Travel independently throughout the clinic (which may include movement from floor to floor); frequent bending, lifting, standing, stooping or sitting for long periods of time may be required

- Communicate and interact in a respectful and professional manner
- Prioritize workload while being flexible to meet the expectations of the daily operations
- Apply principles of logical thinking to define problems, establish facts, and draw valid conclusions

- Effectively operate equipment and communicate on and operate the phone system
- Work independently with minimal supervision
- Travel to other work locations, if required
