Summary:
Under the general supervision of the Director The Comprehensive Cancer Center and in cooperation with the Medical Director assists in coordinating patient navigation activities for select at risk individuals with clinically significant cancers. Provides assistance to healthcare consumers (patients survivors families and caregivers) to help them access and then chart a course through the healthcare system and overcome any barriers to quality care.

Responsibilities:

Participates in coordination of care among multiple disciplines (surgery radiation and medical oncology) including scheduling timing of placement of fiducal markers for radiation and partners with the providers to ensure appropriate timing of surgery after neoadjuvant chemotherapy. Coordinates adjuvant postoperative therapy and assures appropriate data available for new patient visits within multidisciplinary clinics.

Coordination of concurrent chemotherapy and radiation therapy for patients with lung cancer and bladder cancer.

Assists in coordinating referral appointments with health professionals (genetic counselor nutritionist psychiatrist social worker rehabilitation services for continence training sexual counseling fertility counseling and sperm banking as appropriate)

Assists patients in gaining access to support services which may include but not limited to financial assistance with medications and co-pays transportation to treatment referrals to support group and other special programs.

Triages calls from patient/family for services across all product/service lines.

Attends tumor board and ensures patient is scheduled for appropriate follow up labs tests and appointments according to the treatment plan.

Provides support within effective and realistic time frames.

Arranges for second opinions to outside agencies with coordination of medical records radiographic studies and pathology slides.

Provides direction to patients on medical visits and coordinates follow up care such as genetic testing consultations to other specialty clinics

Assists with coordinating appointments for appropriate patient and family education and counseling regarding disease and treatment.

Enhances patients* satisfaction by responding to their support needs during illness.

Participates in ongoing quality assurance activities.

Collects and reports information to ensure compliance with grant and research expectations and reporting requirements if appropriate.

Participates in ongoing education and networking regarding navigation for patients with language barriers. Collaborates with interpreter services to ensure availability/appropriate scheduling or utilization of resources during provider visits.

Participates in outreach programs that target underserved patients.

Other information:

BASIC KNOWLEDGE:

High school graduate or General Equivalency Diploma required.

Analytical skills in evaluating patients in order to plan and provide patient navigation.

Ability to communicate effectively (public speaking skills) with diverse audiences including professional staff patients community media and other service agencies.

Interpersonal skills to allay anxiety elicit cooperation and provide for education of patients/families and to effectively function as a member of the healthcare team to coordinate and discuss patient needs.

Strong organizational skills in order to maintain departmental files records and statistical data.

Proficient typing skills in order to prepare a variety of materials.

EXPERIENCE:

Bilingual (Spanish) preferred.

One to two years* experience working with cancer patients and their families in an informal or formal setting.

Personal unique understanding of the cancer experience.

WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Normal patient care environment with exposure to infectious diseases. Hazards are minimized by adherence to established safety precautions.
