Hear more from our team members about why UnityPoint Health is a great place to work at https://dayinthelife.unitypoint.org Overview Assertive Community Treatment Team Lead RN FTE: 1.0 (Full-time, 40 hours per week) ACT team is responsible for implementing services that provide skill training, supportive care and/or treatment for individuals in recovery for serious mental illness. This position will be responsible for delivery services primarily in the individual's home or community setting. Provide clinical supervision and practice as a member of a multidisciplinary team. This includes involvement in the provision of services following the guidelines for Assertive Community Treatment. Responsibilities * Provides clinical supervision for ACT team staff and monitors quality of care in person and through documentation review. * Completes a pre-screening of all referrals to determine if eligibility criteria are met. Positively interfaces with referral sources, potential clients and their family members through the referral and admission processes. * Provides interventions that recognize that complex nature of individual's resources, strengths and challenges. * Provides on-call interventions covering after business hours, overnights and weekends as assigned. * Recognize early symptoms of physical and mental illness and performs/coaches' staff to perform appropriate interventions in accordance of an individual's crisis plan. * Effectively notifies other supervisors, manager, prescriber, treatment team members, guardians, and other identified individuals if crisis or potential crisis situation. * Develop and utilize crisis planning with team and patients. * Provide therapeutic services as needed for individuals or groups within patients in ACT. * Attends regularly scheduled staff meetings, and in-service trainings. * Coach team members in building appropriate therapeutic approaches for the treatment of patients. * Creates work schedules that provide coverage for 24/7 365 staffing with collaboration of Outreach Manager. * In collaboration with the team psychiatrist or ARNP coordinate, schedule, and administer assessments of client physical and mental health to make appropriate referrals to community physicians for further assessment and treatment. Coordinating psychiatric treatment with medical treatment as appropriate. * Functions as a practicing clinician on the team and provides service coordination (case management) for an assigned group of patients. * Participates in patient-centered comprehensive assessment within 30 days of new admission and as needed on patients as assigned. Qualifications Education: Has a master's degree in a mental health field, including but not limited to nursing, social work, mental health counseling, psychiatric rehabilitation, or psychology Experience: Experience with providing service to individuals with a severe and persistent behavioral health needs. Prior experience providing mental health and substance use disorder treatment. Leadership experience peferred License(s)/Certification(s): Master's Level Social Worker Valid Iowa driver's license. Class D license required. Reliable transportation for use while performing job. Knowledge/Skills/Abilities: Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively. Individual will demonstrate knowledge and skills necessary to provide appropriate care to adult and geriatric patients with severe and persistent mental illnesses. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills. |
