Scope of Position

Acts in interdisciplinary setting to provide quality ambulatory care including extending physicians and independent direct patient care. Acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy and students and other associate hospital personnel. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food, drug-disease state and drug-laboratory interactions. Performs limited physical examination and review of systems, orders and evaluates laboratory and other relevant diagnostic tests. Assumes leadership role in identifying evidence-based opportunities for improvement in patient care and organizational processes. Participates in appropriate hospital and/or departmental committees relevant to practice area.

Position Summary

This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include but are not limited to: assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses. Patient population includes adult, adolescent, and geriatric patients. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising pharmacy residents and students, medical residents and students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff).

Minimum Qualifications

Bachelor's Degree in Pharmacy. Registered Pharmacist. 0 years of relevant experience required. 0-2 years of relevant experience preferred.
