The Maternal Health Case Manager LPN takes responsibility for the delivery of quality and affordability results.

This position is responsible for ensuring the efficiency of the Healthy First Step and Family Visiting program . The Clinical coordinator LPN works closely with the Maternal Health Services R.N. Case managers and the manager of the Maternal health services team. Positions in this function address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities. The LPN CM will need to assess and interpret the members needs and serve as contact for available resources for our members. The job will consist of creating and tracking excel sheets and keeping data from the agencies we partner with for state updates.

Candidate must live in the state of Pennsylvania.

Primary Responsibilities:



  • Make outbound/receive inbound calls to assess member's current health status
  • Utilize motivational interviewing techniques to help member identify and understand their intrinsic goals and motivate members to engage in positive behavior change
  • Identify gaps or barriers in treatment plans
  • Provide patient education to assist with self-management
  • Interact with Medical Directors and team on challenging cases
  • Coordinate care for members under the direction of a Registered Nurse
  • Make referrals to outside sources including pregnancy programs
  • Educate members on Pregnancy and newborn
  • Document and track findings

Required Qualifications:



  • Unrestricted licensure as a Licensed Practical Nurse in the state of PA
  • 2+ years of Case Management and/or Care Coordination experience
  • 2+ years of Maternal and/or Pediatric clinical experience
  • Ability to prove technical knowledge of Microsoft products including Excel and Outlook; ability to learn clinical applications quickly
  • Must have a designated workspace and access to secure high-speed internet via cable/DSL in home
  • Must live in the state of Pennsylvania



Preferred Qualifications:



  • Certified Case Manager (CCM)
  • A background in Managed Care
  • Experience in utilization review, concurrent review and/or risk management

The hourly range for this role is $23.70 to $46.35 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with al minimum wage laws as applicable.
