JOB PURPOSE:

The Uvalde Community Health Worker (CHW) is responsible for supporting and working with clients who are impacted by mental health needs, abuse of alcohol, tobacco and other drugs by providing crisis intervention, advocacy, prevention and education. The Uvalde CHW will also be working with clients in the community to navigate and access Uvalde Community Services and other Community services, and other resources as needed. The CHW supports providers and case managers through an integrated approach to case management and community outreach.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:



  • Responsible for establishing trusting relationships with clients while providing general support and encouragement for wellness needs and services.
  • Conducting health and wellness educational sessions to clients and the community are various topics to include but not limited to mental health, substance use, coping skills, self-care and financial.
  • Create and administer surveys for health and wellness educational sessions provided to clients.
  • Track the number of health and wellness educational sessions provided, number of referrals made, and clients connected to services post referral.
  • Provide ongoing follow-up, basic motivational interviewing and goal setting with clients.
  • Attend regular staff meetings, training and other meetings, as requested.
  • Record client information as needed in Client Management System - Salesforce.
  • Help clients set personal health related goals.
  • Provide referrals for services to community agencies as appropriate for clients.
  • Attend community outreach events as needed to learn about other resources and referral eligibility for services in the community.
  • Help clients connect with transportation resources and provide appointment reminders in special circumstances. Transporting patients based on HHC van policy.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Associates degree. Successful completion of Community Health Worker Certification from an accredited university, federal or state institution (or obtained within 3 months of employment)

EXPERIENCE: 1-2 years of experience working in a community-based setting is preferred. Written and oral fluency in English and Spanish is preferred.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
