Description

The Direct Support Professional is responsible for safety and supervision of our consumers with Intellectual Developmental Disabilities, Behavioral health histories to facilitate healthy and socially acceptable behaviors while living in community-based sites. This position will coordinate with clinical staff, implement residentially based interventions and assist the clients in achieving their clinical assignments. The DSP is responsible for fulfilling the obligations as outlined in the client's program plan. The DSP will prompt or assist, as necessary, to ensure the clients participate in the activities of daily living, in both the home and the community. A commitment to the RHDvalues should be demonstrated as job duties are performed

  • Provide supervision of individuals in accordance with the person-centered ISP; supporting someone to work towards outcomes identified in ISP and other Person-Centered Support Plans
  • Complete required documentation*Administer Medication
  • Provide transportation to consumers*Support consumers in activities of daily living, meal prep, toileting, hygiene, as appropriate
  • Assist in Community Integration Activities and access to community Supports including medical appointments, social security access, etc.
  • Complete all other miscellaneous tasks as assigned by supervisor

Shift availability:

FT Sunday - Thursday 3pm - 11pm (40 hrs)

FT Friday - Monday 3pm - 11pm (32 hrs)

Program Information

Community Passages is a community-based residential program designed to provide a homelike environment for individuals with developmental disabilities and dual diagnoses, many of whom present significantly challenging behaviors and most of whom previously resided at state centers and hospitals. Residents develop life skills, increase their self-esteem, and are empowered to make choices for themselves. Person-centered planning, self-determination, and personalization foster individualized programs in an open and safe environment encouraging consumer empowerment and personal growth.

Requirements

  • High School diploma or GED is required
  • Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job.
  • Prior experience in the human services field, or with IDD or Mental /health Services, preferred.
  • Valid driver's license
  • Subject to employment screenings, including FBI clearances.
  • Subject to a Physical/ TB test.
  • Completion of a 2-week, in-person orientation.
