Department: FERST - Iowa City

Shift: Flexible hours Monday - Friday

Provides Community Support and Supported Employment/Education Services to individuals admitted to the FERST (First Episode Recovery Support Team). CSS functions include helping clients adhere to treatment recommendations, meet basic needs, promote community inclusion, and achieve treatment goals. SEE functions include exploration of employment and education goals, connect to resources needed to achieve employment and education goals, engage community education and employment providers to better understand available resources, and use NAVIGATE curriculum to promote recovery ideals.

Responsibilities

Main Function

  • Implement the NAVIGATE curriculum of care, particularly Community Support Services functions.
  • Provide assertive outreach and creative interventions to develop rapport, treatment adherence, and recovery.
  • Identify unmet basic needs (food, clothing, shelter, healthcare insurance/access, etc) and connect client with resources to meet said needs. Provide direct assistance in securing resources if needed.
  • Support adherence to treatment recommendations by reminding of appointments, arranging for/providing transportation, supporting recommended lifestyle changes by encouraging and/or direct assistance.
  • Explore client hopes and goals specific to recovery and community inclusion. Provide encouragement and direct assistance to promote goal achievement.
  • Documenting all services as required by model, regulators, funders, and best practice.
  • Implement the NAVIGATE curriculum of care, particularly the Supported Employment and Education functions.
  • Provide assertive outreach and creative interventions to develop rapport, employment/education goals, and recovery.
  • Identify client skills, desires, supports, and resources related to employment and education. Connect with resources including request of ADA rights to meet employment and education goals. Provide direct assistance in securing resources if needed.
  • Support achievement of employment and education goals by promoting time management, clear communication with supervisors/teachers, gathering of required work or school supplies, etc by encouraging and/or direct assistance.
  • Explore client hopes and goals specific to employment, education, and recovery. Provide encouragement and direct assistance to promote goal achievement.
  • Documenting all services as required by model, regulators, funders, and best practice.
  • Work in cooperation within the NAVIGATE Interdisciplinary Team.
  • Understand all curriculum for all roles within the team.
  • Cultivate and demonstrate belief in recovery as outlined by the NAVIGATE model.
  • Attend all required meetings and trainings, both internal and external, as assigned by supervisor or required by the model.

Basic UPH Performance Criteria

  • Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Qualifications

Education:

BA degree with at least 30 semester hours in a human services major.

Experience:

At least one year of working with people who have psychosis and/or helping people w disabilities achieve employment/education goals.

License(s)/Certification(s):

Valid driver's license when driving any vehicle for work-related reasons.
