Care Coordination Manager - MATER

The Care Coordination Manager works closely with the the clinical team, reporting to the program director to execute the outreach, referral, intake and care coordination aims of MATER. Builds relationships with area hospitals, physicians/staff, emergency departments, substance use disorder treatment facilities, and other service /treatment programs. Trains and supervises navigators, case managers, administrative staff and Peer Specialists who provide care coordination. Receives client referrals, assigns clients to a navigator, and following intake makes suggestions on caseload assignments based on individual needs. The Care Coordination Manager is responsible for clients' seamless transition into treatment.

Job Description

ESSENTIAL FUNCTIONS:

  • Oversees pre-admission, admission, retention, and discharge/transfer processes, including receiving referrals, assigning clients to navigators for care management, maintaining a waiting list, and facilitating discharges and transfers.
  • Develops systems for tracking data and databases for reporting to state and local authorities, and provides reports to directors.
  • Provide on-going supervision to department
  • Assesses client eligibility for admission into MATER (outpatient and residential) based on level of care assessment, status of medical assistance, authorization for managed care organization clients, and method of payment.
  • Develops and implements a client orientation program.
  • Builds and maintains relationships and referral agreements with agencies/organizations that provide physical and behavioral/mental health care, substance use disorder treatment, housing assistance, and others.
  • Manage supervisees, and train navigators, case managers, and administrative staff in effective techniques to promote client engagement, reengagement, and handle other issues related to receiving substance use disorder treatment.
  • Interacts with co-workers, visitors, and other staff consistent with the iSCORE values of Jefferson.

EDUCATIONAL/TRAINING REQUIREMENTS:

Master's degree in social work or related field from an accredited college/university.

CERTIFICATES, LICENSES, AND REGISTRATION:

  • LCSW, LSW, LPC preferred, but license-eligible individuals will also be considered.
  • Driver's license required.

EXPERIENCE REQUIREMENTS:

  • 2 years' experience in drug & alcohol treatment, health insurance claims, case management, or mental health field required.
  • Supervisory experience preferred.
