The Quality Improvement (QI) Specialist is responsible for all data collection efforts for the Project, gathering and aggregating reports from partners. The Specialist monitors compliance and fidelity to evidence-based programming, conducts on-site observations and data analysis and ensures that all outcome measures are completed as required. This position requires travel throughout the project service areas, the Pomona and San Gabriel Valleys (SPA 3). ESSENTIAL DUTIES AND RESPONSIBILITIES
- Supports and promotes the mission of the Agency: Foothill Family empowers children and families on their journey to achieve personal success.
- Works with Project Director, agency Performance Quality Improvement (PQI) committee, and Youth and Community Advisory Councils to identify appropriate metrics and new collection sources and methods.
- Manages data validation processes and refines data management procedures.
- Provides training to partners and ensures the accuracy and quality of reports that are submitted.
- Collects outcome measure data from each partner and compiles data to present program progress.
- Identifies trends and patterns to measure Project's ability to meet community needs.
- Locates and defines new process improvement opportunities and recommends upgrades or improved reports for better decision-making tools.
- Conducts program evaluation activities in partnership with partners and the Youth and Community Advisory Councils.
- Prepares reports on program goals and objectives, identified performance metrics, and youth outcomes including the completion and timely submittal of the annual report with Director.
- Follows all policies, procedures and legal mandates including HIPAA, client confidentiality, releases of information, protected health information (PHI) and client files.
- Travels between Foothill Family sites for training, meetings, and other in-person interactions.
- Attends in-person meetings and events at various locations within the Los Angeles County and surrounding areas.
- Displays sensitivity to the service population's cultural and socioeconomics characteristics.
- Performs work in a safe manner at all times and ensures that other individuals do not put themselves or others at risk.
- Follows all policies, procedures and legal mandates including HIPAA, client confidentiality, releases of information, protected health information (PHI) and client files.
- Ensures performance and behaviors support the Agency's performance quality improvement (PQI) goals and objectives.
- Reports to work on time and maintains reliable and regular attendance.
- Performs other duties as assigned.
POSITION REQUIREMENTS
- Bachelor's degree in health education, social sciences, public health, education or related field required.
- Knowledge and experience in administration, data analytics, and utilization of electronic reporting and record-keeping systems.
- Ability to administer a complex program by being detail oriented, understanding the systems which help ensure quality of service and accurate record keeping.
- Ability to communicate detailed, technical information, both orally and in writing.
- Local travel between office, school, and community sites required.
- Valid CA Driver's License and maintains insurability on the Agency's auto liability policy (including a minimum of 2 years driving experience after receiving license) and maintains the California state required auto insurance liability limits OR maintains reliable transportation to attend all appointments and meetings as required in the position.
- Must not be excluded, suspended, debarred or otherwise made ineligible on the Federal, State or County Sanctions lists.
- Provides services relating to the administration of public funds or benefits, including eligibility for public funds or public benefits.
