Position Summary:
We have an exciting opportunity to join our team as a Nurse Coordinator - ED Follow up.
In this role, the successful candidate Responsible for planning directing implementing coordinating and evaluating patient care in order to achieve effective patient care and efficient utilization of hospital resources Responsible for the management and quality of care on a daily basis in collaboration with the Nurse Manager Administrative NP Coordinator Director and multidisciplinary team Has a focus on coordination of information about the patient from the referring physician/clinic through the ED and back to the referring physician or coordination of episodic encounters for all patients discharged from the ED with the emphasis on quality efficient and effective care
Job Responsibilities:
- Performs other duties as needed.
- Assure that core measure steps take place.
- Assists with the identification of unit maintenance needs
- Embraces relevant technology and actively improves ones skills through informal and formal learning
- Participates in unit based and hospital wide performance management activities.
- Participates in unit based functioning and task forces; identifies problems and recommend solutions to improve key processes.
- Participates in patient/family education and involves family or significant others in decision making related to plan of care
- Facilitates patient consultations, transfer and/or discharge.
- Calls for case consultation with health care team and activates plan resolves variances
- Coordinate with Nurse Manager and Administrative NP Coordinator for nursing issues
- Collaborates with the ED physician, charge nurse, and staff to prioritize the patient's plan of care, sequencing of diagnostic studies and implementation of treatment and discharge plan
- Communicates directly with referring physicians to gather information about the patient's condition and plan of care and keeps the physician informed about the progress of care
- Functions delegated to RNs are within registered professional nurse scope of practice; this includes, but is not limited to comprehensive telephonic follow-up with patients discharged from the ED and VUC, as well as basic diagnostic testing interpretation, and patient counseling.
- Functions delegated to NP/PAs are within advanced practice provider scope of practice; this includes, but is not limited to those functions performed by the RN, as well as advanced diagnostic testing interpretation, prescribing medication, and placing orders for diagnostic tests that are in-line with the plan of care set forth by the ED or VUC care team.
- Both the RN and Advanced Practice Provider play a key role is assisting the coordination of care between the ED or VUC to the outpatient setting
Minimum Qualifications:
To qualify you must have a BSN Required 3 5 yrs clinical experience Demonstrated excellent communication and interpersonal skills Current NYS RN License Required.
Required Licenses: Registered Nurse License-NYS, Basic Life Support Cert
Preferred Qualifications:
Masters Degree Emergency Department clinical experience Proficient in navigating and troubleshooting multiple technology platforms e g OneDrive SharePoint Microsoft Office Suite Webex and Google Spreadsheets

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $45.63 - $75.00 Hourly. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need.
