Are you a dedicated leader with a passion for delivering high-quality care in home health services? We are seeking an experienced Home Health Clinical Manager to oversee and lead the day-to-day operations of our home care and home health services. As a key member of the leadership team, you will be responsible for driving clinical excellence and ensuring the highest standards of care for our patients.

About Us:

The On Lok Program of All-Inclusive Care for the Elderly at Institute on Aging (On Lok PACE at IOA) is an innovative model of care that is designed to support frail older adults with living- and thriving- in the community for as long as possible. We serve as a primary care team and a health insurance plan, which allows our interdisciplinary team of healthcare professionals to coordinate the right care at the right time, and to meet each patient's unique needs.

Home care is a required discipline on our eleven-member interdisciplinary team. The home care department is responsible for providing personalized, in-home care that supports patients' independence and improves their overall health and quality of life. This ensures that care at home is not just well-coordinated but is an integral part of the interdisciplinary team's care plan.

Role Overview:

As the Home Health Clinical Manager, you will be responsible for overseeing the clinical operations and ensuring the effective delivery of home care and home health services. You will lead a dedicated team of nurses and caregivers guiding them in best practices while ensuring compliance with regulatory standards. Your leadership will ensure that our patients receive the highest level of care, all while optimizing operational efficiency.

  • Oversee daily operations of the home care department.
  • Provide leadership and direction to clinical staff, ensuring adherence to best practices and regulatory standards.
  • Build and sustain a team of competent caregivers who provide quality and compassionate care.
  • Collaborate with interdisciplinary teams to coordinate care and improve patient outcomes.
  • Monitor performance metrics and implement quality improvement initiatives.
  • Ensure compliance with federal, state, and local regulations governing home health services.
  • Mentor and support staff to ensure a high level of patient care and satisfaction.
  • Provide collaborative leadership for the PACE program at IOA with the management team.

ESSENTIAL JOB FUNCTIONS:

  • Manages and oversees the daily operations and services provided by the home care department, which encompasses skilled home health services and custodial care.
  • Makes certain the department has adequate staffing, oversees schedules, and adheres to a competency program. Holds responsibility for staff development, training, recruiting, retention, and employee engagement.
  • Manages and oversees the overall clinical operation of home health services. This includes operationalizing and overseeing IOA's home health license compliance and coordinating with home health contracted vendors. Works closely with clinic leadership to ensure close communication and effective coordination of medical care. Monitors and facilitates program compliance with federal, state, local and plan-level requirements.
  • Ensures best clinical practices and that nurses and caregivers are performing within their full scope and providing consistency and continuity of care.
  • Holds responsibility for building a highly effective home care department that fully integrates custodial home care and home health services into the overall care plan.
  • Ensures clarity of procedures, standards of care, and collaboration within and between the other departments.
  • Adheres to SEIU 2015 terms and conditions. Participates in collective bargaining, management meetings, or other Union meetings, as needed.
  • Promotes quality of care by participating in Quality Assurance Improvement Plan (QAIP) activities, initiatives, process improvement plans, committees, etc. Provides clinical leadership through oversight of infection control best practices.
  • Monitors and helps develop budget for areas of responsibility.
  • Participates in after-hours rotational on-call duties.
  • Actively participates in and encourages actions that promote good public relations with participants, their families and friends, visitors, and the community.
  • Other duties as assigned.

REQUIRED QUALIFICATIONS:

  • Active, unrestricted licensure as a Registered Nurse in California
  • A bachelor's degree in nursing from an accredited program by the National League for Nursing.
  • At least three years of experience in the past five years in a home health agency, primary care clinic or health facility, at least one year of which was in a supervisory or administrative capacity.
  • At least three years of management experience.
  • At least one year of experience with frail older adults.
  • Current BLS certification
  • Demonstrated skills leading and motivating teams and individuals. Demonstrated understanding of the importance of relationship building.
  • Familiarity with the application of Information Technology (IT) to clinical care, including patient care record-keeping, quality assurance, outcomes assessment and pertinent privacy requirements.
  • Ability to work within an interdisciplinary team with initiative, imagination, resourcefulness and flexibility.
  • Interest and ability to work in a multilingual, multicultural environment.

PREFERRED QUALIFICATIONS:

  • Master's degree in Nursing (MSN) or a closely related field
  • Familiarity with Title 22 Home Health regulations
  • Bilingual English/Cantonese.

PHYSICAL REQUIREMENTS:

  • Ability to lift, transfer, push/pull, maneuver, and reposition 35 lbs.
  • Ability to reach, bend and walk.
  • Five finger manipulation.
COMPENSATION
  • Range: $ 128,824.28 - 149,436.16 USD Annual

This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.

This range does not include any additional equity, benefits, or othernon-monetarycompensation which may be included.
