Career growth and development Diverse and collaborative working environment Affordable and comprehensive benefits package $5,000 sign-on bonus Position Summary: The Health Behavior Specialist is the primary interface for patients to access services such as behavioral health screening, assessment, brief treatment, consultative services, education and engagement, linkages, outreach and advocacy, and peer/family counseling and support. Responsible for the delivery of patient centered mental health, substance abuse, and health behavior care to assigned patients in the community as well as designated primary care or acute care sites, working under the supervision of the Director of Behavioral Health Program or designee, with clinical oversight by a licensed MD, DO, or APC with behavioral certification, as part of clinical services in the Office of Population Health. Additionally, this position will work with physicians, case managers, community health workers, mid-levels, primary care providers, or inpatient medical teams and patients and family members to promote qualitative and efficient use of health care services through the patient's behavioral changes and modifying the care delivery system. Competencies: Education: Master's degree from a program accredited by the Council on Social Work Education or an equivalent organization approved by the Council on Social Work Education. Salary Range: Minimum 27.55 per visit - Maximum 45.50 per visit. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
