HIM/Medical Records Specialist

Hours for this position are: Monday-Friday 7am-3:30 pm

Responsible for the timely completion and submission of the Birth Certificate to vital statistics and completion of proof of paternity according to the established guidelines and statues. Other responsibilities include the performance of miscellaneous clerical functions including: prepping, scanning, quality control of medical records, Inpatient/Outpatient/ER record processing, filing, record retrieval, loose reports, processing and telephone answering. Outpatient analysis.

Essential Job Duties:

  • Checks birth certificates for completeness and accuracy in a timely manner. Proofreads birth certificates and makes corrections as necessary.
  • Accurately completes and submits Birth Certificates to the office of vital statistics within 5 days of birth.
  • Acts as a resource for new parents in L&D with birth certificate and paternity acknowledgement questions.
  • Educate unwed parents on the voluntary Paternity Acknowledgement program and ensure compliance with SC POP goal to have 55% or higher Paternity Acknowledgement rate.
  • Makes flexible appointments with parents and is willing to accommodate the parent's schedule in order to complete the birth registration process.
  • Attends ongoing seminars for timely completion and accurate submission of birth certificates and proof of paternity as required by the state.
  • Logs all births onto the Birth Certificate log on a daily basis. Updates the baby names in Cerner.
  • Answers and/or faxes inquiries from DHEC (local and state offices).
  • Enters the Hearing Screenings in a timely manner.
  • Facilitate the outpatient record completion process by assigning signature and dictation deficiencies to the appropriate practitioners using the online chart completion module.
  • Perform quantitative/qualitative analyses of outpatient medical records ensuring compliance with Joint Commission, the Department of Health and Human Services, CMS and other regulatory agencies.
  • Works on chart audits daily and completes monthly spreadsheet for Medical Records Director.
  • Coordinates Chart Audits completed by Outpatinet Departments, and reports results and compliance to HIM Director
  • Re-analyze medical records to determine if deficiencies have been corrected and update the deficiency entries in the online chart completion module.
  • Picks up discharged records daily and reconciles them against the appropriate discharge list. Follows up with supervisor on any missing records.
  • Accurately preps medical records utilizing the correct document type barcodes.
  • Accurately Prepares Paper charts for scanning according to Department guidelines
  • Accurately split and merge documents in the quality control application.
  • Accurately scan records to the correct patient MRN and Encounter in the EMR meeting the established productivity and accuracy standards.
  • Accurately scan color images using the appropriate scanner settings when necessary.
  • Accurately performs quality checks of all records scanned on a daily basis.
  • Check and resolve any issues with the scanning software to ensure records with errors get resolved in a timely manner.
  • Routinely provides scanner maintenance, which includes daily cleaning of the scanning equipment.
  • Handles ROI requests accurately and timely when ROI staff is absent.
  • Set up Patient Portal access for Patients and Proxy's. Obtian Proxy consent for adult to Audult proxy access and scan Consent into the Medical record prior to authorizing access
  • Assist Patients and/or Proxy's with problems accessing the patinet portal, refer to Technical Support as needed.
  • Follow up with Physician offices to obtain correct fax numbers that appear in the PCP failed fax reports. Have number updated in system & fax the failed report to correct number.
  • Assist Coding Supervisor obtain missing documenation that is prohibiting coding. Search medical record to find documentation. If unable find, calls Provider office to obtain documentation.
  • Works with all hospital departments and physician offices to provide education on medical record documentation/completion requirements and ensure compliance.
  • Assist with pulling and reviewing records eligible for the Annual Medical Record destruction.
  • Promptly answers the telephone courteously within three rings.
  • All other duties as assigned.

Qualifications

Education requirements: Associate degree in business or related major required.

Work experience requirements: Two years general office experience, preferably in a healthcare setting in Health Information Management.

Required skills, knowledge and abilities: Knowledge of medical terminology is required. Previous medical records experience is required. Word processing skills required. Excellent communication skills required. Must be able to pay attention to complicated work procedures and remember them.
