JOB SUMMARY

The Education and Employment Mental Health Specialist/Case Manager IV reports to the Supervising Personal Service Coordinator and is responsible for assisting clients in obtaining desired education through access to vocational training schools, community and local colleges, universities and internet based programs. The Education and Employment Mental Health Specialist/Case Manager IV will also provide assistance with the client's individual needs for job development, job seeking skills, job retention skills, on-the-job assessment, training, and support.

ESSENTIAL JOB RESPONSIBILITIES



  1. Under the supervision of the Supervising Personal Service Coordinator, assist in the accomplishment of the case management plan with clients by screening, evaluating, identifying, and implementing specific case management activities as identified in the Individual Service and Support Plan (ISSP) to support clients in achieving their goals.
  2. Maintain appropriate case recording documentation as per the Rehab option Medi-Cal guidelines, case recording, and other contract guidelines.
  3. Attend weekly supervision meetings with the Supervising Personal Service Coordinator.
  4. Provide direct services, including case management, to clients. Such services may include group counseling, individual, and or other forms of direct service as outlined in the Individual Service and Support Plan (ISSP) and Plan of Care.
  5. Maintain and submit all documentation and billing as it relates to client services in a timely fashion.
  6. Plan, coordinate, and facilitate services for Severely Mental Ill (SMI) clients.
  7. Screen and evaluate potential clients for the identification of case management, rehabilitation, residential, and other services and activities.
  8. Provide orientation, career planning, and employment education development services to clients.
  9. Provide individual and group guidance to clients assisting them in the development of skills necessary to maintain employment and continue educational goals.
  10. Assess clients for work readiness.
  11. Develop individual education/employment or training plans with clients.
  12. Provide classroom educational instruction or training plans with clients.
  13. Develop employment and or work experience opportunities in the local community for clients.
  14. Develop and maintain outside supportive referral services for clients.
  15. Provide counseling to clients to assist in development of job relation skills.
  16. Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of clients.
  17. Provide the Program Director with timely reports on any unusual occurrences that might impact the overall program operation or the health and safety of clients or employees.
  18. Complete the minimum 40 hours training annually per Turning Point training requirements.
  19. Think and act quickly and efficiently in emergencies.
  20. Will drive on company business and transport consumers as needed.
  21. Physical presence in the office is required.


ESSENTIAL JOB REQUIREMENTS



  1. A Bachelor's Degree from an accredited or regionally approved college or university in the area of social work, psychology, vocational rehabilitation, criminal justice, or a related field.
  2. A minimum of one (1) year of experience in the management of rehabilitation for difficult to serve families with barriers to successful independent community life is preferred.
  3. Knowledge of best practices regarding the treatment of clients with Severe Mental Illness (SMI) and Dual-Diagnosis individuals.
  4. Communicate effectively in written and spoken English.
  5. Work effectively with diverse ethnic, cultural, religious, socioeconomic, and professional groups of people.
  6. The ability to be certified in emergency first aid procedures.
  7. Intermediate computer skills in Microsoft Word, Excel, Outlook and PowerPoint, Office 365, Teams, Zoom and other communication/meeting platforms.
  8. Possess a valid driver's license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required.
  9. The ability to pass a criminal background check.
  10. The ability to pass a pre-employment drug screen and TB test.

      • Monolingual: Class 130 ($21.13 - $27.81/hour)
