Your Role on the Team The Patient Care Coordinator will assist patients in the coordination of their care through scheduling appointments, serving as a liaison between patients and clinicians, facilitating communication, interpreting hospital policies, procedures, and services. The Patient Care Coordinator will also be responsible for various levels of patient satisfaction and issues, including but not limited to sensitive and complex matters. The incumbent will also be responsible for meticulously tracking and coordinating patient visits and referrals, ensuring timely follow-up appointments are scheduled in accordance with a given clinician's protocols as well as patient needs. The incumbent will also educate patients about their rights in accordance with California state law. What It Takes to be Successful
