Description Clinical Pharmacy Specialist at Providence Milwaukie Hospital in Milwaukie, OR. This position will also float to 2 other Providence locations: Portland Medical Center and St Vincent Medical Center in Portland Oregon. Full-Time/Day Shift $1,000 Sign-on Bonus for eligible external candidates who meet all conditions for payment - this is in addition to the fantastic benefits and compensation package offered by Providence that begin on your first day of employment. The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients. The responsibilities of the clinical pharmacist will predominantly fall into the following areas: (1) Patient Care - The Clinical Pharmacist Specialist is responsible for providing collaborative practice services in collaboration with the healthcare team. (2) Population Management - The Clinical Pharmacist Specialist may utilize registries for chronic diseases and reports/tools to identify target populations/patients and enhance therapeutic outcomes through disease/medication education and drug therapy optimization. (3) Preceptor / Educator - The Clinical Pharmacist Specialist will provide education to the healthcare team and patients in written and verbal formats that include formal and informal presentations and publications. The Clinical Pharmacist Specialist is a drug therapy resource to physicians and other members of the health care team. The Clinical Pharmacist Specialist may participate in the selection and training of pharmacy learners. Providence caregivers are not simply valued - they're invaluable.
